NEW PATIENT QUESTIONNAIRE

Your New Patient Questionnaire will be sent to you electronically once you have made your initial appointment. The New Patient Questionnaire should be completed before the day of your appointment.  It will be sent through our HIPAA compliant Patient Portal. Please be sure to set up your Patient Portal username and password as soon as you receive the invitation from our office.  The portal will be the primary electronic mode of communication once you become a patient.

MEDICAL HISTORY TIMELINE

Please be sure to complete the medical history time line prior to your appointment as well.  This will be sent along with your New Patient Questionnaire. Please put some time and thought into this so that all of the pertinent details of your case can be reviewed at your initial appointment.

*** Dr. Birkmeier-Fredal does require that her patients continue to have a Primary Care Physician (Adult and Pediatric).  She does not act as your Primary Care Physician/Pediatrician.
