Your Health Timeline is one of the most important tools in helping us understand your journey and uncover the root causes of your health concerns. Taking the time to fill it out thoroughly will give us a clearer picture of what you’ve been through and how we can best help.It may be helpful to gather old medical records as you complete this. Start your timeline from the point when your current issues became problematic, and be sure to include:

Looking at how your body has responded to past treatments gives us valuable insight into what’s working, what’s not, and how we can create the most effective path forward for you. Take your time with this—it will be well worth it!
What to Expect at The Restorative Medicine Center
Initial Consultation
During this time, the doctor will:
- Review your new patient questionnaire
- Discuss your medical history, symptoms, past treatments, and lab work
- Go over your current medications and supplements
We encourage you to be open and honest during this process—this helps us create the most effective treatment plan for you. No judgment, just support.
Once all the necessary details are gathered, the doctor will share her clinical impressions and outline a personalized care plan tailored to your needs. If lab work is recommended, she will walk you through the process and next steps.
Your initial appointment will last about 60 minutes and can take place in person, over the phone, or via Zoom—whatever works best for you!
Follow-Up Consultations
Follow-up visits typically occur 2-8 weeks after your initial consultation, depending on lab results. The second visit is also 60 minutes, while subsequent visits are generally 30 minutes.
During follow-ups, the doctor will:
- Assess your progress and any changes in symptoms.
- Review your treatment plan and adjust therapies as needed.
To streamline this process, please complete your Follow-Up Questionnaire before each appointment. Proper follow-up is essential for monitoring your progress and ensuring your treatment is effective. As your health improves, follow-ups will become less frequent.
At the end of each visit, you will receive a summary of your appointment with clear instructions for next steps. This summary will be provided in paper form for in-person visits or accessible through the Patient Portal under the "Visit Summary" tab.
Root CausE Triad testing
Your testing journey begins at your first appointment and is tailored to fill in any knowledge gaps based on previous tests you’ve had.
We typically start with LabCorp bloodwork to assess immune function, key nutrient levels, and genetic predispositions. In most cases, LabCorp testing is covered by insurance, though this depends on your specific plan.
If needed, we may also recommend specialized out-of-pocket testing to uncover deeper drivers of immune imbalance. These tests can provide critical insights, and the sooner we have this information, the better we can tailor your treatment. However, we always prioritize testing based on your individual needs and budget.
Some of the specialized tests we may suggest include:
Mosaic MycoTox – Detects current environmental mold exposure.
DNA Connexions Lyme Panel – A PCR test for vector-borne infections.
Stool testing or urine organic acid testing – Helps assess internal fungal overgrowth.
Since every person is unique, we determine testing on a case-by-case basis to ensure the best approach for your health.

Fees & Policies
At The Restorative Medicine Center, we are committed to providing transparent and comprehensive care tailored to your needs. Our consultation process is designed to ensure you receive personalized attention, whether in person, over the phone, or via video. We offer detailed information about our fees and office policies to help you navigate your healthcare journey with confidence.
Consultation Fees
Effective January 1st, 2025:
Initial Consultation: $10/minute
- Example: 60-minute in-person visit = $600
Follow-Up Consultations: $9/minute (available in-person or remote)
- Less than 10 minutes: $90
- 20 minutes: $180
- 30 minutes: $270
- 45 minutes: $405
- 60 minutes: $540
Please note: The final cost is based on the total time spent during your appointment, which may be more or less than the scheduled duration.
Appointment Time
You will be billed for the full duration of your appointment. Dr. B values your time and works hard to stay on schedule, so we kindly ask that you do the same. Your total appointment time covers all aspects of your care, including chart reviews, test result analysis, and post-appointment summaries. The good news? Most of this time is spent directly with Dr. B, giving you a thorough, personalized experience where all your concerns are addressed.
We appreciate your partnership in making each visit as efficient and meaningful as possible! Medical letters are also a billable service.
Dr. Birkmeier does not assist with disability or FMLA filings; please contact your Primary Care Doctor for these services.
INSURANCE COVERAGE AND REIMBURSEMENT
The Restorative Medicine Center does not accept or bill insurance. We can provide a superbill for you to submit to your insurance carrier, but reimbursement is not guaranteed.
LabCorp, located in our office, accepts most insurances, but we encourage you to verify coverage with your provider.
Cancellations and No Shows
A 3 business day notice is required for cancellations or rescheduling. Less notice incurs a fee equal to the missed appointment cost.
